INDOOR SOCCER LEAGUE REQUIRED FORMS
League Deadlines
- » All players must be identified on the current team roster and have completed the online NSC Liability Waiver within 48 hours of game start time in order to be eligible for league play.
- » Exception: Rosters are frozen prior to playoffs. No player additions are approved after these dates:
- » Session I: November 18
- » Session II: January 15
- » Session III: March 14
- » Ineligible players will result in a forfeit.

» Current official team rosters are available to the referees at the monitor’s desk in the event of a challenge. See rules.

- » You will need your team ID number to complete the online roster. The team ID number is provided via email upon acceptance. Contact Michelle Johnson with any questions.
- » Enter each player’s name and jersey number.
- » Direct your youth parents/adult players to complete the online NSC Liability Waiver (links are below); they will need the team ID number to complete this task. No paper waivers accepted.
- » Youth parents/adult players will provide their address and date of birth on the online NSC Liability Waiver.
- » Best of all! You can monitor the progress of your youth parents/adult players by selecting View Roster from the online roster menu. The WV column stands for waiver. Each time a youth parent/adult player completes the online waiver, an X is displayed in the column to the left of the player’s name, in addition to player’s date of birth and address.

League Balance
- » The team fee balance must be paid before the first game.
- » If you are paying by cash, office hours are Monday-Friday, 9 am-5 pm. The main office is located in the Administration Building. Enter the building from parking lot B off Davenport Street and proceed to level 2/Plaza Level.
Proof of Age
- » Proof of age is required for all youth players prior to first game.
- » Adult players must be able to produce ID in the event of a challenge. Ineligible players will result in an automatic forfeit. See rules.
- » Proof of age includes any form of Minnesota State ID, driver’s license, passport, school ID with birth date, birth certificate, and/or player pass.
