5V5 HOLIDAY CLASSIC REQUIRED FORMS

1. Complete the online tournament roster using our Team Management Tools (available later this year)

  • »  Preliminary roster id due December 17.
  • »  Roster changes may occur until December 26 at 1600.
  • »  You will need your tournament ID number to complete the online roster. The team ID number is provided via email upon acceptance.
  • »  First, enter each player’s name, jersey number and position. Note:
    • »  Positions may change.
    • »  Guest player designation (GP) does not apply in this tournament
  • »  Second, direct your parents to complete the online NSC Liability Waiver; they will need the team ID number to complete this task. No paper waivers accepted.
  • »  Parents will provide their player’s address and date of birth on the online NSC Liability Waiver.
  • »  Best of all! You can monitor the progress of your parents by selecting View Roster from the online roster menu. The WV column stands for waiver. Each time a parent completes the online waiver, an X is displayed in the column to the left of the player’s name, in addition to the player’s date of birth and address.

2. Direct parents/adult players to complete the online NSC Liability Waiver (available later this year)

  • »  Waivers due December 17.
  • »  Provide parents with the tournament ID number for this task.
  • »  Players are not permitted to participate in the tournament without a completed NSC Waiver on file.

3. Proof of age required for all youth players

  • »  Player pass, copy of birth certificate, passport or driver's license are required at final team check-in.
  • »  Player passes from previous seasons may be used.

4. Required Hotel Lodging, if applicable

  • »  If your team is staying in a hotel, view our Official Host Hotels for a list of required hotels, rates and amenities.
  • »  Email jeen@nscsports.org with the name, city and phone number of your hotel as back-up emergency contact. Be sure to include your tournament ID number in the subject line.

Final Team Check-in 

  • »  Location: Monitor office, located directly outside the sports hall.
  • »  Representative must check in team one hour prior to team's first game
  • »  Proof of age required for all youth players. Player pass, copy of birth certificate, passport or driver's license accepted.
  • »  Adult teams should be prepared to show proof of age if challenged.
