Sanctioned Soccer Tournament Required Forms
Once your team is accepted into the tournament, you will receive an email with a link to your Team Dashboard.
In your dashboard, the following will need to be completed:
- Copy the “Share Invite” link on the left side below your team name and send this out to each team members’ parent or guardian. They will input their information and agree to the player waiver before joining the team.
- This is REQUIRED for all players to be listed on your tournament roster.
NSC Online Player Waiver
NSC Sanctioned Tournaments are Youth Events – All waivers must be completed by the parent or legal guardian of each player (even if the player is 18+yrs).
In order to complete a waiver, the parent or guardian must select the link provided by their team manager, fill in their child’s information, and agree to the waiver. This step is required in order for the player to be listed on the tournament roster.

US Club Soccer Teams (USC)
USC Teams must provide the following documents for all sanctioned events (Not required for Minnesota USC teams).

US Youth Soccer Teams (USYS)
USYS Teams must provide the following documents for all sanctioned events (Not required for Minnesota MYSA teams).

United States Specialty Sports Association (USSSA)
USSSA Teams must provide the following documents for all sanctioned events.

Canadian Teams

Player Passes
Current season player and coach passes MUST be submitted prior to the required forms deadline.
- Player passes are used as proof of age for the tournament.
- All players on a team MUST be from the same governing body, including guest players and coaches.
- Contact your club registrar or tournament registrar for more information.
Canadian Teams
- Canadian player books, Provincial player cards, and/or FIFA affiliate player passes are accepted.
**NEW THIS YEAR – physical, laminated player passes are NOT required at the start of each game, but MUST be submitted prior to the required forms deadline. ALL teams must be prepared to show proof of player passes at any time throughout the tournament, IF eligibility is in question.
- Proof of player passes can include the following: physical passes, digital passes, or PDF version of passes. These documents can be provided via smartphone, if needed.
File Upload Instructions
In the center your Team Dashboard, click the ‘COMPLETE SURVEY’ link next to “File Uploads Incomplete” to upload your required documents (see Official Rosters & Travel Documentation tab for more information).

Please try to condense your documents to one file, if possible. There will be extra file upload space provided, if needed.
Guest Players
Guest Player Rules
Guest players must be registered with USYS, US Club Soccer or FIFA affiliate and possess a current youth player pass.
A ‘guest player’ is any player not identified on your current USYS/US Club Soccer or Provincial/Club roster.
Guest players must be registered with a FIFA affiliate (USYS, USC, etc.) that matches the team they are guest playing with and possess a current youth player pass. (Ex: US Club Soccer players may NOT guest play on USYS teams, and vice versa).
Teams may not exceed six (6) guest players.
Guest Player Permission/Loan Forms
MN Youth Soccer Assoc. (MYSA/USYS Member)
- A Guest Player Permission Form is only required IF the guest player is from a different USYS State Association. (Ex: A player has a USYS pass but is not issued through MYSA).
US Youth Soccer (USYS)
- A USYS Guest Player Permission Form IF the guest player is NOT listed on the official roster.
US Club (USC)
- MN Teams – A US Club Guest Player Loan Form IF the guest player is NOT from the same club.
- Non-MN Teams – A US Club Guest Player Loan Form IF the guest player is NOT listed on the official roster.
United States Specialty Sports Assoc. (USSSA)
- A USSSA Guest Player Loan Form for anyone NOT listed on the Official Roster.
Girls Academy (GA)
- A GA Guest Player Permission Form for anyone NOT listed on the Official Roster.
MLS Next
- Guest playing is NOT allowed. Anyone not listed on your official roster is not eligible to play.
Final Team Check-in
All teams are required to send one (1) team representative to check-in 90 minutes prior to the team’s first game (unless otherwise noted on the specific tournament webpage).

Rosters will be frozen at Final Team Check-in. The team representative will receive copies of the official tournament roster for presentation to the referee at the start of each game.

- Youth players currently registered with both a sanctioned youth and adult league may be eligible to play. Player pass will reflect proper documentation.
- All players must meet the age requirements and be listed on the team roster.
- Players may play up or down, if age eligible.
- Roster sizes:

- U9-10 playing 7v7 may roster and dress up to 14 players.
- U11-U12 playing 9v9 may roster and dress up to 16 players.
- U13-U15 playing 11v11 may roster and dress 18 players for games.
- U16-U19 playing 11v11 may roster 22 and dress 18 players for games.

- Teams may NOT over roster and cannot exceed the six-guest player limit.

- If your roster exceeds the six-guest player limit, contact your state association for information regarding reclassification as a tournament team. The six-guest player limit is not negotiable; it is part of the tournament sanctioning. If your team is reclassified as a tournament team, forward the documentation to the tournament contact. Guest players may play at the same level, or a higher level of competition offered by the tournament.

- Females may play on male teams; males may not play on female teams.
- A jersey must be provided to the guest player by the host team.
