Sanctioned Soccer Tournament Required Forms

1. Complete online Tournament Roster using the specific event Team Manager Account.

2. Parents/Guardians complete the online NSC Player Waiver using NSC Event HQ.

3. Submit/Upload Official State Roster & Travel Documentation (if applicable).

4. Provide/Upload Player Passes.

Team Manager Account

Please see the tournament webpage for specific Team Manager Account Links.

Manage the team roster online to add, drop, or edit player information for the event.

  1. Access the account by using the Team ID number received after being accepted into the event. Be sure to click on “Create Pwd” if it is your first time accessing the account.
  2. Verify Concussion Training – Check the box indicating concussion training has been completed. (This is a requirement of the state of Minnesota Statute 121A.37. The U.S. Center for Disease Control and Prevention has a free, online course entitled “HEADS UP” is accessible by the link here.)
  3. Click “Manage Players” button to add players to the NSC Roster. This will allow parents/guardians to complete the online NSC Player Waiver.

NSC Online Player Waiver

1. Log in to an existing NSC Event HQ Account (Don’t have an account? Click on the step-by-step instructions link below).

2. Click Customer Tools, then Manage My Participants to list the player under the parent/guardian NSC Event HQ Account.

3. Click into Customer Tools, Complete Waiver, Filter to the Event, Enter the Team ID, and follow the prompts to complete the player waiver.

NSC Sanctioned Tournaments are Youth Events – All waivers must be completed by the parent or legal guardian of each player (even if the player is 18+yrs). A parent or legal guardian (of 18+yrs) must set up the NSC Event HQ account. Questions? Click on the step-by-step instructions below.

US Club Soccer Teams (USC)

USC Teams must provide the following documents for all sanctioned events (Not required for Minnesota USC teams).

  • Official State Roster

US Youth Soccer Teams (USYS)

USYS Teams must provide the following documents for all sanctioned events (Not required for Minnesota MYSA teams).

  • Official USYS State Roster
  • Notification to Travel Document
    • Contact your USYS state association for your teams Official State Roster & Notification-to-Travel documents.

United States Specialty Sports Association (USSSA)

USSSA Teams must provide the following documents for all sanctioned events.

  • Official USSSA State Roster
  • Certificate of Insurance

Canadian Teams

Canadian Teams must provide the following documents for all sanctioned events.

  • Official Province/Club Roster
  • Team Notification-to-Travel Document
    • Contact your province/club for your teams Official Provincial Roster and Notification-to-Travel to a USYS sanctioned tournament.

Player Passes

PHYSICAL, LAMINATED PLAYER PASSES ARE REQUIRED FOR All NSC SANCTIONED EVENTS.

1. Prior to the Event – All Teams must submit current Player / Coach Passes to the Registrar for Verification and Proof of Age.

  • This can be a PDF or photocopy of the passes. Submit the document through NSC Event HQ. See the “File Upload” tab for instructions.

2. During the Event – All teams are required to present current physical laminated Player / Coach Passes to the referee at the start of each game (NO Electronic Passes will be accepted).

  • All passes for each team must be from the same governing body. EX: All must be USYS or US Club. Teams cannot intermix passes within the same team.

File Upload Instructions

Securely submit all required documentation (required “forms”) by using the “File Upload” feature in the NSC Event HQ portal.

1. Log into an NSC Event HQ account. (See NSC Online Player Waiver tab for instructions on how to create an account)

2. Click into “Customer Tools” menu, select “File Upload”. Filter to the event, and enter the event specific Team ID.

3. Upload files and documents!

  •  Only files with the following extensions are allowed for uploading: “png”, “pdf”, “gif” and “img”.

Guest Players & Pool Players

Guest Player Rules

Guest players must be registered with USYS, US Club Soccer or FIFA affiliate and possess a current youth player pass.

  • A ‘guest player’ is any player not identified on your current USYS/US Club Soccer or Provincial/Club roster.
    • Guest players must be registered with a FIFA affiliate (USYS, USC, etc.) that matches the team they are guest playing with and possess a current youth player pass. (Ex: US Club Soccer players may NOT guest play on USYS teams, and vice versa).
    • Team Roster Limit – 6 Guest Players per team.

Guest Player Permission/Loan Forms

  • MN Youth Soccer Assoc. (MYSA/USYS Member)
    • A Guest Player Permission Form is only required IF the guest player is from a different USYS State Association. (Ex: A player has a USYS pass but is not issued through MYSA).
  • US Youth Soccer (USYS)
    • A USYS Guest Player Permission Form IF the guest player is NOT listed on the official roster.
  • US Club (USC)
      • MN Teams – A US Club Guest Player Loan Form IF the guest player is NOT from the same club.
      • Non-MN Teams – A US Club Guest Player Loan Form IF the guest player is NOT listed on the official roster.
  • United States Specialty Sports Assoc. (USSSA)
      • A USSSA Guest Player Loan Form for anyone NOT listed on the Official Roster.
  • American Youth Soccer Organization (AYSO)
      • An AYSO Guest Player Permission Form for anyone NOT listed on the Official Roster.

NSC Pool Player Portal

This Portal is designed to allow individual players interested in guest playing in the event, enroll into the pool to be seen by coaches seeking additional players to supplement their roster for the tournament.

Players – How to Enroll

1. Log into an NSC Event HQ account. (Click here for step-by-step instructions to create an account.)

2. Click Customer Tools, then Player Pool Sign Up.

3. Filter the event and follow the prompts to join the pool.

      • Interested coaches will contact you directly by email.
      • Pool players may only participate on one team.

If you are picked up by a team:

      • Make sure you have your printed, physical player pass for the event.
      • Provide the coach with your player code for release of your information to the team roster.
      • Remove your name from the player pool for this event by logging in to Event HQ, find your event pool and select “Leave Pool”

Coaches – Access to Pool Players

1. Log into your event specific Team Manager Account and select player pool.

2. Search for players by division, level and/or position, and FIFA affiliation (FIFA affiliation must match your team).

3. Click the player’s first name for contact by email.

      • If the individual agrees to participate on your team, the player will provide his/her player code necessary for release of information to your roster.
      • Return to Team Manager Account player pool. Click the green check mark before the player’s name; enter the player code, which will add the player’s contact information and liability waiver electronically to your roster.
      • Complete Guest Player Form (if applicable).
      • Plan with your guest player to secure the physical player pass in advance of Final Team Check-in or have your guest player check in prior to your arrival at Final Team Check-in.

Final Team Check-in

All teams are required to send one (1) team representative to check-in 90 minutes prior to the team’s first game (unless otherwise noted on the specific tournament webpage).

      • Location: Welcome Center building (1750 105th Ave NE Blaine, MN 55449)
      • Time: 90 minutes prior to the team’s first game

What is Needed?

      • Physical laminated player and coach passes are required at check-in and before each game.
      • Any outstanding documents needed for the event.

Rosters will be frozen at Final Team Check-in. The team representative will receive copies of the official tournament roster for presentation to the referee at the start of each game.

Pro Tips:

      • Anyone (Coach, Manager, Parent, etc.) can check in for the team at Final Team Check-in.
      • Arrange player passes in the same order as your roster to ensure you have all passes and for a faster check-in.

Roster Guidelines

Please see the tournament webpage for specific event guidelines.

      • Youth players currently registered with both a sanctioned youth and adult league may be eligible to play. Player pass will reflect proper documentation.
      • All players must meet the age requirements and be listed on the team roster.
      • Players may play up or down, if age eligible.
      • Roster sizes:
          • U9-10 playing 7v7 may roster and dress up to 14 players.
          • U11-U12 playing 9v9 may roster and dress up to 16 players.
          • U13-U15 playing 11v11 may roster and dress 18 players for games.
          • U16-U19 playing 11v11 may roster 22 and dress 18 players for games.
      • Teams may NOT over roster and cannot exceed the six-guest player limit.
          • If your roster exceeds the six-guest player limit, contact your state association for information regarding reclassification as a tournament team. The six-guest player limit is not negotiable; it is part of the tournament sanctioning. If your team is reclassified as a tournament team, forward the documentation to the tournament contact. Guest players may play at the same level, or a higher level of competition offered by the tournament.
      • Females may play on male teams; males may not play on female teams.
      • A jersey must be provided to the guest player by the host team.
